Puppy Questionnaire

We sincerely appreciate your request for information about our Kuvasz. We breed kuvasz because we love the breed. It is very important to us that our puppies are healthy, happy, and well cared for. Placing a puppy or dog in a home is the easy part; placing a puppy or dog in the right home is a much more difficult task. We need to have information abut you, your family and your lifestyle in order to make a better selection of the puppy that will become a part of your family.

We ask you take a few minutes to complete the following client information sheet in detail. If you wish to provide us with any addition information please add it to the final field.

The purchase of a puppy should be well thought out. You are making a commitment for the life of the puppy and should have as much information as possible to making the decision to buy.
